ABSTRACT

OBJECTIVE: In most cases, the postoperative course of a chronic subdural hematoma (CSDH) is good, but CSDHs sometimes remain after the inserted catheter is removed, and the outcomes of such remnant hematomas are unclear. As oral streptokinase-streptodornase (OSS) has anti-inflammatory and hematologic effects, we assessed the effects of OSS on remnant CSDHs through a retrospective analysis of clinical data. METHODS: This study included 101 patients with traumatic CSDH who had remnant CSDH after burr-hole trephination with the closed drainage system between October 2009 and December 2012. We assessed the clinical outcomes, remnant CSDH volume, and recurrence rate from computed tomography scans in order to evaluate the effects of OSS. RESULTS: No significant differences were found in the changes in remnant hematoma volume between the OSS-treated and non-treated groups (p=0.531). The reoperation rate because of was 5.4% (2 patients) in the OSS-treated group and 6.3% (4 patients) in the non-treated group. The number of patients requiring reoperation did not differ between the groups (p=0.658). CONCLUSION: OSS treatment was not associated with a significant reduction in the remnant CSDH volume or recurrence rate after burr-hole drainage. Thus, there seems to be no basis for using OSS in CSDH patients.

ABSTRACT

It was a prospective and randomized study of 124 consecutive patients scheduled for elective surgery at AFIC/NIHD. Patients were assigned to cold crystalloid cardioplegia, warm blood cardioplegia and cold blood cardioplegia groups. Patients were comparable with respect to preoperative data and cardiac status. Biochemical measurements, metabolic indices and post operative morbidity and mortality were evaluated before and after the operation. CPK, CK-MB, AST, LDH and ALT measurements were significant among different groups. Their concentrations were highest after cold crystalloid cardioplegia and least after warm blood cardioplegia. The warm blood cardioplegia appears to confirm best myocardial protection, cold blood cardioplegia lies in the center and cold crystalloid ardioplegia exerts sub-optimal degree of myocardial protection. This study provides guidelines for the selection of a better cardioplegia solution giving optimal results

ABSTRACT

To evaluate the clinical incidence and outcome of patients with pericarditis after streptokinase therapy for acute myocardial infarction, 113 patients were prospectively studied during acute myocardial infarction [AMI]. Pericarditis was defined as the presence of a pericardial friction rub during the hospital course. Only 8% of patients developed a rub during AMI, a low% compared with that in the prethrombolytic era. A pericardial friction rub more often occurred in the setting of an anterior wall AML Patients when compared to those without, a pericardial friction rub had lower ejection fraction [40 vs. 54%, p=0.003]; worse regional left ventricular function; higher in-hospital mortality [22 vs. 10%, p=0.05]; a higher frequency of power failure [100 vs. 54%]; a higher frequency of anterior wall location of the AMI [78% of cases, p=0.003]. Therefore, although the frequency of a pericardial friction rub was low [8%] compared with that in the prethrombolytic era, its occurrence denotes more extensive myocardial damage with a worse clinical outcome. Perhaps with successful reperfusion, transmural myocardial necrosis is prevented and with it the development of pericarditis. Cardiac tamponade did not occur clinically in any patient who developed a pericardial friction rub

Os fármacos fibrinolíticos representam um importante coadjuvante terapêutico para o cirurgiäo vascular em casos de trombose venosa e arterial. Há contudo muitos aspectos contraditórios. Seu uso em trombose venosa profunda näo tem aceitaçäo plena, apesar de alguns relatos de bons resultados. Quanto às patologias arteriais, sendo a correçäo cirúrgica difícil e limitada para as artérias de pequeno calibre, a fibrinólise pode representar uma alternativa ou um coadjuvante terapêutico. Deve ser lembrado que os efeitos colaterais induzidos pelos fármacos fibrinolíticos podem ser graves e limitar seu uso, havendo também suas contra-indicaçöes. Além disso, esta terapia exige uma adequada estrutura de UTI e laboratório para sua aplicaçäo

ABSTRACT

O objetivo deste estudo foi verificar a vantagem da associacao de enzimas a antibioticoterapia no tratamento da doenca pelvica inflamatoria (D.P.I.). Foram incluidas nesta pesquisa dois grupos de 20 pacientes cada um, com diagnostico de doencas pelvica inflamatoria (D.P.I.). O 1o. grupo foi tratado somente com antibioticoterapia, enquanto o 2o. foi tratado com associacao de antibiotico (ampicilina) e enzimas. Foram observadas as seguintes vantagens deste 2o. tipo de tratamento:resolucao mais rapida dos sintomas dolorosos cura clinica em tempo prolongado; facilitacao da penetracao do antibiotico no foco infeccioso inflamatorio quando associado as enzimas. A tolerancia as enzimas foi satisfatoria, nao sendo observados quaisquer efeitos secundarios que pudessem ser diretamente relacionados a administracao desse farmaco. Entretanto, devido a casuistica reduzida, recomendamos que novas pesquisas sejam efetuadas principalmente um estudo duplo-cego

ABSTRACT

Se presenta un estudio comparativo en la respuesta de hipersensibilidad demorada frente a 4 antigenos universalmente recomendados. Tuberculina (TU), Candidina (CA), Streptokinasa/Streptodornasa (SK/SD) y virus de parotiditis (PA) con otros antigenos de uso menos comun tal como Antigeno-Respiratorio-Mixto (ARM) y lisado de Staphylococcus Aureus 80/81 (LS). De igual manera se estudio la respuesta al Dinitroclorobenceno (DNCB). La poblacion probada fue de 49 personas normales con edades entre 18-23 anos. Los resultados mostraron que la reactividad para ARM es de 91,8%, para LS 91,8%, para Parotiditis 6,1%. Los estudios con DNCB permiten aconsejar su uso solo en casos muy especiales. El trabajo presentado sugiere una normalizacion en la lectura y el uso de 4 antigenos con la mas alta frecuencia de positividad dentro del grupo probado asi: ARM,LS,CA y TU

ABSTRACT

1. A comparative studies of the effects of Varidase and Urokinase on the experimental hyphema produced by the injection of rabbits auto-blood were performed in 20 eyes. 2. Subconjunctival injection of each enzymes were perfomed 1 hour after production of hyphema, and thereafter repeated once daily for 10 days. 3. Varidase increased the rate of absorption of hyphema, but Urokinase had no effect. 4. No side reactions were observed following the subconjunctival injection of these enzymes.

ABSTRACT

A Case of Eales' disease in a 36 years, old R.O.K. officer, male, is presented in this report. This patient has been suffered from Eales' disease for these 3 years. Although the recovery from this disease was occurred two times previously, it was resulted as Complete blindness of O. D. due to the Complications of this disease and poor visual acuity of O. S. on account of marked vitreous opacity and retinal hemorrhage in spite of intensive treatment with anti-tuber culous agents, steroids, varidase and subconjunctival injection of 3% Sodiumchloride solution.
